Understanding root causes is essential to solving problems. Healthcare is no different. The social determinants of health are at the root of population health. Recently healthcare systems have stepped up and have started to look at the social determinants of health. Going beyond the typical “fix” a patient and send them on their way cycle, they are looking at the social causes at a societal level to improve health across all populations.

In this presentation two Community and Population Health experts will review their efforts to impact their local community through the social determinants of health.

Learning Objectives

1.       Understand social determinants of health and how they play a role in population health.

2.       Review how healthcare systems are stepping up to address population health and improve the social determinants of health.

3.       Describe how healthcare supply chains can support the mission to impact and improve the social determinants of health.

Max Kursh, MS, is Director for Community and Population Health at Cooper University Health Care in Camden, NJ. He is responsible for outreach, education and community benefit efforts that support Camden and neighboring communities.  He recently led projects increasing access to: COVID vaccines and education; clinical services for military and first responders; and social determinant of health resources.  Prior to joining Cooper, Max worked as a job developer at the University of Delaware’s Center for Disabilities Studies creating vocational training opportunities for young adults with disabilities.  He is a graduate of the University of Delaware and holds an MS in Health Promotion.  He is also a trained Lean Six Sigma Green Belt. 

Danielle Santiago-Roach, MS, the Community Health Worker Manager at Cooper University Health Care in Camden, NJ. She is responsible for managing Cooper’s Community Outreach worker program that helps mitigate social barriers for residents in and around Camden, NJ. She has been with Cooper for 11 years and has experience in program development, health coaching, and HIV counseling. She is a Drexel University and Walden University graduate and holds an MS in Health Education and Promotion.
